I had copied an article I found last summer that defined the seven stages of Alzheimer's and had "symptom" lists for each one that gave me some ways I could identify changes/losses in my brother that were a more factual, behavioral guide..

I have also given my brother a mini mental test every couple of months because I usually feel I am not able to make assessments of how he is doing without being influenced by my own emotions. He actually went about 18 months before his score began to change. During that period there were up and down swings of not more than 2 points. Then he had a significant drop and has remained at same point again with not more than a swing of 1 point in either direction. I keep a copy of the test itself along with the additional "parts" and a scoring sheet that has the dates and scores of each time I gave him the test.

There's also the FAST Scale (Reisberg Functional Assessment Staging Test).
I wanted one that incorporated the "Instrumental Activities of Daily Living" and also the "Activities of Daily Living" -- because care providers will ask about those things -- but didn't find one anywhere online, so finally, I just made my own spreadsheet.

That is a very difficult question since every dementia patient is different . It depends a lot on what part of the brain is affected… My man was diagnosed with MIXED TYPES … There is some great information to be gained with on line meetings. The Alzheimer’s Association help line will head you in the right direction …
