Rayc97, my wife also talks to people on tv. She will stand close to the tv, wave her hands and talk loudly n a disapproving tone. I prefer that behavior to having her sit on her bed staring at nothing or walking the hallway while mumbling to herself. My recommendation is, as long as it is not too exaggerated, let her talk to the tv. No harm done.

My mother has recently watched TV shows and thinks they are real and not a film or program. We have had to stay away from watching any violence or cop shows. We have spending a lot of time on the Halmark channel but even then she believes the woman or man is going to kill the other (love interest). She does warn them or us of what is "going to happen". We reassure her that it is a move or show and that no one is getting killed. There was a period of time after watching a police show that she believed the criminal was going to get her or her family. After trying to reassure her that the show was a story, I told her the policy caught the criminal and that he/she was in jail. She didn't need to worry about it any longer. I told my father and sisters to back me up. After a few days, she stopped worrying.
I talk to the TV, all my fur babies, birds, bugs, anything in view, with absolute passion and determination, regardless the object of my interest. Living or not, ppl or not, moving or not, real or not. Anything that peaks my interest. No shame in my game. I'm quite content within my head. Until im not. Then, the bitch is back! U gotta laugh.
My husband also confuses tv and advertisements with reality. Really can watch and movies or programs. Sometimes sitcoms are ok. He will also talk al the time during programs, preventing me from hearing them. I do read books a lot.
